Dr. Ronald Bloomfield is a United States Immigration and Naturalization approved civil surgeon. We will assist the applicant with the successful completion of the medical requirements for securing a green card . We do all necessary testing on site and have all required vaccines in our office. We will provide you with the I-693 and I-693 supplemental form at no charge.

We perform examinations most days of the week. We are often able to examine patients the same day they call for an appointment.

We also accept proof of vaccination done elsewhere. The Columbus Health Department, located at 240 S. Parsons Avenue, 614-645-7945, offers vaccines at reduced cost for those 18 and younger. We are not allowed to accept insurance or personal checks for any of our services. Payment is due day of service. We gladly accept all major credit cards.
